Job Description:
A company is looking for a Community Health Worker to join their
Case Management team.
Key Responsibilities
Assist members in following an outcome-based plan of care and
address current needs
Outreach to identified members to solve barriers affecting their
care
Monitor care and services to ensure quality care delivery in
appropriate settings
Required Qualifications
High school diploma or GED
CHW certification from an approved Community Health Certification
Program or two years of equivalent work experience
1+ years of experience in care coordination and navigation
Bilingual in English and Spanish or Creole is a plus
AA or Bachelor's degree in social science or health-related field
is a plus
