POSITION SUMMARY The Inpatient RN Care Manager works to enhance clinical outcomes and patient satisfaction while managing the cost of care. They facilitate patient care, conduct utilization management, and coordinate care progression and transition. This role requires a thorough understanding of the RN scope of practice, state and federal regulatory requirements, and best practices for patient care.

• Assessment: Conduct comprehensive patient assessments and communicate with attending physicians to ensure appropriate care and utilization of resources.
• Planning: Develop safe and effective discharge plans, considering the patient's clinical condition, resources, and the risk of readmission.
• Implementation: Coordinate care progression, identify barriers to care, facilitate transitions to different care settings, and ensure timely resolution of issues.
• Evaluation: Develop and evaluate case management plans, monitor patient care outcomes, and identify sources of barriers to care progression.
• Communication/Collaboration: Collaborate with interdisciplinary teams, community providers, payers, and patients/families to ensure safe and effective care plans and transitions.
• Professionalism: Maintain respect for all patients, uphold patient privacy and confidentiality, and demonstrate concern and caring for all customers, regardless of their diagnosis or socioeconomic status. POSITION REQUIREMENTS A. Education: Bachelor of Science degree in nursing preferred, with a minimum of an Associates in Nursing required. B. Qualifications/Experience: Minimum of one to three years of hospital or related experience required. Familiarity with outside provider networks preferred. C. Special Skills/Knowledge: Bilingual language skills preferred (Spanish), basic computer skills, current California Nursing license, current Basic Life Support (BLS), and certification in Case Management preferred.
