Job Description:
A company is looking for a Care Manager, RN to coordinate
patient care and manage individual care plans.
Key Responsibilities
Develop and implement client-centered Individual Care Plans
Coordinate and facilitate patient care through assessment and
evaluation
Act as a patient advocate regarding insurance coverage and
financial assistance
Required Qualifications
Registered Nurse license required
Bachelor's degree in nursing or a related health services field
preferred
Two years of experience in utilization management
Three years of clinical experience, preferably in case
management
Certification as a Certified Case Manager (CCM) is preferred
