Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and barriers to care, developing personalized
care plans
Coordinate care between members, families, and providers to ensure
appropriate services
Provide ongoing follow-up and education to members and their
families on healthcare options and resources
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
Registered Nurse (RN) licensure in Florida or Compact State
Licensure
Experience in OB/NICU and behavioral health preferred
