Job Description:
A company is looking for a Care Manager (RN) to develop and
facilitate complex care management activities for members with
physical needs.
Key Responsibilities
Evaluate member needs and barriers to care, recommending
personalized care plans
Coordinate care between members, families, and providers to ensure
effective service delivery
Provide ongoing monitoring and education to members and their
families on healthcare options and resources
Required Qualifications
Degree from an Accredited School of Nursing or a Bachelor's degree
in Nursing
2 - 4 years of related experience in care management
RN - Registered Nurse - State Licensure or Compact State Licensure
required
Behavioral health experience preferred
Must be located in the eastern time zone
